If You Can't Urinate, The Cause May Be Urethral Stricture

What is urethral stricture?

The connecting tube between the tip of the penis and the bladder, which allows the urine to be thrown out of the body, is called the urethra. Urethral stricture, on the other hand, is a condition that causes urine to be unable to be removed comfortably from the bladder and thus urine to accumulate in the bladder. Stenosis occurs as a result of injury or damage to the urethra and surrounding tissue. For example, in an injury to our skin, conditions such as stretching and thickening of the area that is the wound are observed. In a similar way, there is a stretching and thickening condition in the urethra injury. When this stretching and thickening clogs the urethra cavity; urine cannot pass through the urethra and urine accumulates in the bladder.

What are the symptoms of urethral stricture?

The bladder emptying begins to impairment always gives a feeling of fullness, but the urine is not fully excreted due to the existing stenosis. We can list its main symptoms as follows:

  • Weakening of urine flow
  • Feeling pain when urinating
  • Feeling like you have to urinate constantly

When the disease progresses; as urine produced in the kidneys cannot flow into the bladder, it accumulates in the ureters and  it may cause kidney damage by causing enlargement of the kidney pelvis in the following processes. Also, urinary tract infections such as cystitis can develop and lead to serious problems.

How to diagnose urethral stricture?

First of all, a detailed medical history is taken from the patient. Then, a physical examination and evaluation accompanied by endoscopic and radiological examinations are performed. After these stages,  final diagnosis can be made.

What is the treatment of urethral stricture?

Because of the urgency of the condition caused by urethral stricture that urine remains in the bladder, catheter can be placed after urination or over the projection of the bladder on the skin. Thus, the emergency situation is eliminated by emptying the bladder, and the patient is relieved.

After a final diagnosis of urethral stricture has been made, surgical alternatives may be preferred to eliminate this condition. One of these methods is the urethrotomy, which is often performed. The purpose is to expand the urethra. In patients deemed appropriate, this method is performed by expanding the part where stenosis is present, either by laser or mechanically.

In patients who are not suitable for a urethrotomy operation or who have not achieved success, the cause of stenosis can be eliminated and expanded with open surgery, which we call uretroplasty. In some cases, the surrounding tissues can be used to repair the urethra.
